«Amount owed» is what is earned minus what has been paid. It can go NEGATIVE, and that is not an error being hidden: it means more has been paid out than that month produced — an advance on the next one, or a mistake worth looking at. It is left EMPTY when there is no rate, because then nobody knows what is owed, and writing a number there would look like an answer.
The «Rate» box is left EMPTY when the rate changed during the month: that period has no single rate, and picking one would show a number that, multiplied by the hours, does not give the amount next to it.

Hours people measured themselves. There are no screenshots behind them: what you have is the day, the length, what they say they did, and how that day already looks. Approving adds the time to their pay; refusing needs a reason, and they can read it.

Add hours for someone
Use this when the work happened but nobody declared it. The entry is added already approved — there is nothing to approve, the decision is the entry — and it goes into their pay. It is recorded in the log as added by you, not as approved, so it stays distinguishable from hours they asked for.
